As Care Coordinator, you will be instrumental in the Carer
Coordination, Client Management, Account Management and Administration of
hundreds of elderly residents for our leading Home and Community Not For Profit
provider.
Working closely with Registered Nurses, you will ensure
residents are monitored and reviewed for increases in services and changes to
care plans.
Pivital in this role is the coordinating (rostering) of staff
and management of Carer workforce(s) using the in house roster management
system. You will have excellent time management skills, the ability to work
autonomously as well as be part of a larger team.
Ideally, you will have previous Home Care Services experience
along with Rostering/Coodination experience. You will also have a current valid
police check, excellent oral and written communication skills and be computer
literate.
