Due to an internal promotion, our client, a leading Aged Care provider is
looking for a highly enthusiastic CM to support to various facilities located in
and around Melbourne's South Eastern suburbs.
 
Forming part of an independent team, you will  work closely with Facility
& Clinical Managers in providing comprehensive assessment and care plans,
maximising ACFI, recommending changes to care and assist in ensuring
documentation for residents care meets requirements for continuity of
care.
 
 
This really is an amazing opportunity with variety and autonomy. Its a
senior role within the organisation, one regarded as critical. You will need to
work collaboratively with all levels of management, clinicians, carers,
residents and their families to facilitate effective clinical outcomes.
 
 
Essential requirements:

• RN Div 1 registration with APHRA
  
• Working knowledge of ACFI standards and documentation
  
• Current National Police Check
  
• Have a good understanding of continuous improvement, OH&S and Risk
  Management
  
• Australian Citizen or Permanent Resident
