Reporting to the Facility Manager, we are looking for Care Support
Coordinators who will work along side Clinical Managers but with a focus on
developing, panning, implementing and evaluating assessments and care plans that
are of best practice and reflect resident needs and improve outcomes for
residents.
Its an interesting role, which includes providing a mentoring role in
guiding and educating staff in the implementation of care plans to ensure that
resident care needs are actively put in to place.
Due to the importance of these roles you must possess the following:
- RN Div 1 registration with APHRA
- Excellent understanding of ACFI standards and Documentation
- Three years experience in post graduate nursing
- A working knowledge of the needs of the elderly persons residing with the
care facility
- Strong written and verbal communication skills
- Excellent problem solving, assessment and care planning skills
- A great Attitude
- A current valid police record check
